Decision Replay Template

A structured record of a consequential AI-supported decision: proposal, evidence, permission, boundary, human decision and outcome.

Use this when

A decision matters enough that the organisation should be able to explain it later—to the owner, a colleague, a client or itself.

Decision Replay is an accountability and learning instrument. It is not a post-hoc justification tool and it must not be retrospectively cleansed.

Capture the moment

Record the decision at the point it is made. Do not rely on a reconstructed story once context, evidence and uncertainty have faded.

Keep the record honest

If evidence is later challenged, record that challenge as a later event. Do not rewrite the original decision as though the organisation knew then what it knows now.

A useful Replay leaves the organisation more capable of seeing the next decision clearly—not more skilled at defending the last one.
